At Solera-Med, we understand that the privacy and security of your protected health information (PHI) is of utmost importance. Other Uses and Disclosures: We may use and disclose PHI for other purposes with the patient’s written authorization, including for research purposes.

Right to Access: Patients have the right to inspect and obtain a copy of their PHI.

Right to Request Restrictions: Patients have the right to request restrictions on the use and disclosure of their PHI.

Right to Request Amendments: Patients have the right to request amendments to their PHI.

Right to an Accounting of Disclosures: Patients have the right to receive an accounting of disclosures of their PHI made by us.

Right to File a Complaint: Patients have the right to file a complaint with us or with the Department of Health and Human Services if they believe their privacy rights have been violated.
